About the Role
Make a difference in people’s lives while shaping how we deliver health and quality in supported living.
We’re looking for a qualified nurse with a passion for improving health outcomes for people with learning disabilities and complex needs. You’ll combine hands-on nursing expertise with a focus on quality, medication safety, and person-centred practice.
What you’ll be doing:
- Leading on complex health support and ensuring personalised support plans are accurate and up to date.
- Building strong links with hospitals, GPs, and specialists, making sure people have positive healthcare experiences.
- Overseeing medication audits, spot checks, and staff competency assessments.
- Promoting health and wellbeing initiatives, including diet, nutrition, hydration, mobility, and epilepsy care.
- Monitoring and acting on changes in physical and mental health needs.
- Using data and audits to drive continuous improvement in health quality.
What we’re looking for:
- NMC-registered nurse (RGN, RMN, RNLD).
- Experience supporting people with complex health needs.
- Strong understanding of medication management and audits.
- Confident working with multidisciplinary teams and external health professionals.
- Passionate about person-centred care and empowering people to manage their own health.
